BS Equine Health Self Declaration Form
All competitors attending our British ShowJumping shows must fill out the above Equine Health Self Certification Form and return to kirsty@ianstarkequestriancentre.co.uk 24 hours prior to arrival. Should a horse arrive at the venue without the form having been submitted their entry will need to be forfeited and the horse should not be allowed on to the venue grounds.
